extends RuntimeException

The session mutation did not report a confirmed commit. This is not absence, success, or permission to repeat an OAuth exchange. An HTTP adapter must report temporary unavailability (no-store), without issuing/clearing session cookies or claiming logout. Start a new login after a failed callback.
